New Patient Information
Our virtual GLP program is designed for patients who want convenient, physician-guided weight management care without scheduled office visits, phone calls, or video appointments.
Care is provided through an asynchronous telehealth model, meaning you communicate with Dr. Garza and our clinical team by email. This allows you to receive ongoing support, medication guidance, refill management, and treatment adjustments on your schedule.
We are a self-pay medical practice. Consultation and membership fees are not billed to insurance. Patients with prescription insurance may still be able to use their benefits for eligible medications, and we assist with prior authorizations when appropriate. We help patients access discounted compounded medication options through trusted pharmacy partners, without adding a GLP-1 medication markup from our practice.
Our practice focuses on GLP-1 medications for weight management. We do not prescribe phentermine, appetite suppressants, or other controlled substances.
Program Fees
New Patient Enrollment: $249
This one-time fee includes your medical review and first 3 months of care.
Ongoing Membership: $59/month
Medication costs are separate and vary based on the medication, dose, pharmacy, insurance coverage, and available savings programs.

What’s Included
The $249 new patient fee includes:
Initial medical review
Prescription management
Direct email access to Dr. Garza and the clinical team
Medication guidance and treatment planning
Lifestyle support and accountability
Prior authorization assistance when appropriate
Access to discounted compounded medication options
Supportive prescriptions when medically appropriate
Optional lab ordering when clinically indicated
Labs and medications are billed separately. Lab testing is not required for every patient and is ordered only when appropriate.
After the First 3 Months
Patients who continue treatment remain active through the $59/month membership.
This includes:
Ongoing medication management
Refill review and prescription renewals
Dose adjustments when appropriate
Quarterly progress reports
Email-based clinical support
Safety and effectiveness monitoring
Prior authorization assistance when applicable
Supportive medication prescriptions when appropriate
There are no contracts. Patients may cancel their monthly membership at any time.
Prior Authorization Policy
Prior authorization assistance is available only for enrolled patients.
We submit prior authorizations at no additional charge when the medication is being used for an FDA-approved indication. Off-label medication requests may require a $25 processing fee per submission.
Insurance approval is determined by the insurance company and cannot be guaranteed.
How to Get Started
Step 1: Complete the Intake Form
Complete the New Patient Intake Form and upload your government-issued ID. If you have prescription insurance, please upload your insurance card as well.

Step 2: Staff Review
Our team will review your information. Typical processing time is 3–5 business days, although holidays and high-volume periods may take longer.
To avoid delays, please avoid sending multiple emails about the same request.
Step 3: Pay the Consultation Fee
After your intake form is reviewed, we will send a secure Clover payment link for the $249 consultation fee. Please check your spam or junk folder.
The sender may appear as Plano Weight Management Medical Associates or Dr. Garza.
Step 4: Begin Treatment
Once your intake form and payment are complete, we will email you with next steps. This may include medication options, insurance review, prior authorization support when appropriate, treatment instructions, and prescription details.
If you already know which medication or pharmacy you prefer, please include that information in your intake form or email us after submitting it.

Eligibility
Basic eligibility requirements include being age 18 or older, meeting appropriate medical criteria, and having no contraindications to GLP-1 treatment.
Each patient is reviewed individually based on medical history, weight history, treatment goals, and safety considerations.
