For Existing / Established Patients Continuing Their GLP-1 Treatment
After your first 3 months of care, continued prescription renewals through our virtual GLP-1 program require two steps: submitting your Quarterly Progress Report and maintaining an active monthly membership.
We do not automatically enroll you in the monthly membership/subscription without your approval.
① Submit Your Quarterly Progress Report
Please complete your 3-month check-in form here:
https://form.jotform.com/232113546586154
This report is required every 3 months so we can review your progress, medication response, side effects, hunger and food noise control, and overall response to treatment. Based on this information, we will determine the most appropriate next dose or treatment plan.
② Maintain an Active $59 Monthly Membership
The $59 monthly membership covers professional services related to ongoing care, including refill review, dose adjustments when appropriate, progress review, and continued email-based support.
The easiest way to explain this fee is that it is similar to the cost of monthly office visits for refill review and ongoing treatment support.
This fee does not include medication costs.
You may cancel at any time by emailing our office. There are no long-term commitments.
Once your Progress Report has been submitted and your monthly membership is active, we will review your progress and issue updated prescriptions promptly, if clinically appropriate.
Refills Are Not Automatic
You are responsible for requesting each refill when needed. To help avoid any lapse in medication, we recommend requesting your refill at least 2 weeks before you expect to run out.
If you have remaining refills on an active prescription, please contact the pharmacy directly and provide the RX number printed on your medication box.
If we have already submitted a prescription for a higher dose, please note that each dosage is considered a separate prescription and will have a different RX number.
Please note that prescription renewals may be assigned a different RX number than your previous prescription, especially if the dose or medication has changed. Always use the most current RX number when contacting the pharmacy about a refill.
Please contact our office to request a new prescription if:
• You have no refills remaining on a dose you with to continue
• You need a dose adjustment
• You want to change medications
• You do not have an active prescription on file
Refill requests cannot be approved until the following requirements have been completed:
• Your Quarterly Progress Report has been submitted
• Your $59 monthly membership is active
• Any outstanding balance on your account has been paid
All patients receiving GLP-1 prescription management through our practice (regardless of medication and dose) must keep their monthly membership active and current. Missed or inactive membership payments may interrupt ongoing care, prescription renewals, refill services, and access to treatment through our office.
